Ogilvie Syndrom = Intestinale Pseudoobstruktion Extrem überblähter Dickdarm ohne klar erkennbare Ursache
Diagnostik

- Sehr charakteristisches Bild
- Sono Abdomen
- Gastrografineinlauf
- Coloskopie + Dekompression
Therapie

[Bearbeiten]- Medikamentöse Maßnahmen:
- Mestinon (nicht anzuwenden bei Patienten mit Muskeldystropie z.B. Duchenne)
- Gabe von Neostigmin (ACh-Esterasehemmer)
- Flüssigkeitsgabe intravenös
- Darmrohr und Einläufe
- Magensonde bei Erbrechen
- Dekompression mittels Darmspiegelung
- Künstlicher Darmausgang am Coecum
Darmrohr
[Bearbeiten]Donnertropf
[Bearbeiten]- Der Donnertropf besteht aus den Bestandteilen
- Metoclopramid (z.B. Paspertin 2ml/10mg Amp.)
- Neostigmin (z.B. Prostigmin 1ml/0,5mg Amp.)
- und eine entsprechende Trägerlösung (Ringer, physiologische Kochsalzlösung, G5%) mit einem Volumen von 500ml.
Zusammensetzung Der Donnertropf wird zusammengestellt aus jeweils 6 Ampullen der oben aufgeführten Bestandteile und der Trägerlösung.
- 6 Amp. Metoclopramid (12ml/60mg)
- 6 Amp. Neostigmin (6ml/3mg)
Gesamtvolumen: 530ml
Infusionsgeschwindigkeit: 40-80ml/h, (entsprechend ca. 6-13 Stunden)
Der Darmtonus soll maximal angeregt werden
Medikamentöse Alternativen:

- Erythromycin
