Anatomie des Dickdarmes

Ogilvie Syndrom = Intestinale  Pseudoobstruktion Extrem überblähter Dickdarm ohne klar erkennbare Ursache

Diagnostik[Bearbeiten]

  • Röntgen Abdomen oder Thorax im Stehen
  • Sono Abdomen
  • Gastrografineinlauf
  • Coloskopie + Dekompression

Therapie[Bearbeiten]

Übersicht[Bearbeiten]

  • Medikamentöse Maßnahmen:
    • Mestinon (nicht anzuwenden bei Patienten mit Muskeldystropie z.B. Duchenne)
    • Gabe von Neostigmin (ACh-Esterasehemmer)
    • Flüssigkeitsgabe intravenös
  • Darmrohr und Einläufe
  • Magensonde bei Erbrechen
  • Dekompression mittels Darmspiegelung
  • Künstlicher Darmausgang am Coecum

Darmrohr[Bearbeiten]

Donnertropf[Bearbeiten]

  • Der Donnertropf besteht aus den Bestandteilen
    • Metoclopramid (z.B. Paspertin 2ml/10mg Amp.)
    • Neostigmin (z.B. Prostigmin 1ml/0,5mg Amp.)
  • und eine entsprechende Trägerlösung (Ringer, physiologische Kochsalzlösung, G5%) mit einem Volumen von 500ml.

Zusammensetzung Der Donnertropf wird zusammengestellt aus jeweils 6 Ampullen der oben aufgeführten Bestandteile und der Trägerlösung.

Gesamtvolumen: 530ml

Infusionsgeschwindigkeit: 40-80ml/h, (entsprechend ca. 6-13 Stunden)

Der Darmtonus soll maximal angeregt werden

Medikamentöse Alternativen:

  • Ceruletid (Takus)
  • Erythromycin
